Tax Form 1095-C availability

Please note the availability of the 2022 1095-C tax forms for faculty and classified in the additional information below.
Faculty 1095-C forms for tax year 2022 will be sent to employee's mailing address by March 3, 2023. Form 1095-C is provided by the employer for all individuals who had medical coverage for any period during 2022 through Anthem Blue Cross Blue Shield.
Classified 1095-C documents are now available in Benefitsolver under ‘Personal Documents'’ for all employees to download. Employees who did not consent to receive the document electronically will receive the document via mail within 7-10 days. Classified Staff please note: Form 1095-C is provided by the State benefit office for all individuals who had medical coverage for any period during 2022. Covered dependents are listed in Part III of the form for Cigna enrollees only. Kaiser Permanente enrollees will find their covered dependents listed on Form 1095-B (provided by Kaiser Permanente). A copy of Form 1095-B is available to Kaiser Permanente enrollees in their online account under ‘My Documents’ within the ‘My Account’ drop-down list. There will be two 1095-B forms posted. One is for January – June, and the other is for July – December.
You are able to file your tax returns without the 1095-C document.
