Required Annual Conflict of Interest Disclosure - Due 3/31

New process/form for Conflict of Interest Disclosures. 2024 Annual Disclosure Due Date - March 31, 2024
Most non-classified employees and certain classified employees must submit an annual Conflict of Interest Disclosure Form. This year we are rolling out a new electronic process for completing your annual disclosure. We will be sending a disclosure form to applicable employees through our Governance, Risk and Compliance system, MetricStream, in the form of a survey during the last week of February. We will keep the disclosure period open through the end of March to give everyone time to complete their disclosure form.
Additional information will be provided via email regarding the annual disclosure requirement and how to access and complete the disclosure form in MetricStream. You can also access this information on our Conflict of Interest website (https://www.mines.edu/compliance/conflict-of-interest/). While training is not required, we plan to have a few training sessions available to provide information and answer any questions you may have about the disclosure requirement or process. More information to come regarding trainings.
The disclosure form survey will come from MetricStream (donotreply@metricstream.com) with the subject line Respond To Survey [xxxxxx] : 2024 CONFLICT OF INTEREST DISCLOSURE. Please note that this is a valid email containing a link to give you access to your disclosure form. The system will require you to use your SSO credentials to log in.
